Client Overview:
We are a devoted family seeking assistance for our elderly couple, both in their 80s, who are experiencing advanced dementia and sundowning. Our beloved family members require specialized care to manage their cognitive decline and behavioral symptoms effectively. As their family, our utmost priority is to ensure their safety, comfort, and well-being as they navigate the challenges of dementia. We are seeking skilled nursing assistance to provide specialized memory care, environmental modifications, and support for managing behavioral symptoms to enhance the quality of life for our elderly couple during this challenging time.

Project Overview:
We are seeking an experienced nurse to join our caregiving team on a contractual basis, with a focus on providing specialized care for our elderly couple living with advanced dementia and sundowning. This role involves providing memory care, implementing environmental modifications to promote safety and comfort, and offering support for managing behavioral symptoms, fostering a nurturing and supportive environment for our beloved family members.

Responsibilities:

Memory Care: Provide specialized memory care interventions tailored to the unique needs of each spouse, focusing on preserving their cognitive function, stimulating memory recall, and promoting engagement in meaningful activities. Utilize reminiscence therapy, validation techniques, and personalized routines to enhance their quality of life and maintain their dignity and sense of identity amidst cognitive decline.
Environmental Modifications: Assess the home environment and implement modifications to promote safety, orientation, and comfort for our elderly couple with dementia. Make adaptations such as installing handrails, removing tripping hazards, labeling key areas, and creating calming and familiar spaces to reduce confusion, agitation, and disorientation, enhancing their ability to navigate and function independently within their home.
Behavioral Symptom Management: Work collaboratively with our elderly couple’s healthcare team to develop and implement individualized care plans for managing behavioral symptoms associated with dementia, such as agitation, aggression, wandering, and sundowning. Utilize non-pharmacological interventions, behavior management techniques, and therapeutic activities to address their behavioral challenges effectively, minimize distress, and promote a sense of calm and security.
Family Education and Support: Offer guidance, education, and emotional support to our elderly couple’s family members as they navigate the complexities of caring for loved ones with advanced dementia. Provide training on dementia care strategies, communication techniques, and coping skills to empower family caregivers in meeting the unique needs of their spouses, while also attending to their own well-being and managing caregiver stress and burnout.
Collaboration: Collaborate closely with other healthcare professionals involved in our elderly couple’s care, including physicians, geriatricians, psychologists, and occupational therapists, to ensure coordinated and holistic support. Communicate regularly with the interdisciplinary team to share information, discuss care goals, and address any emerging needs or concerns, promoting continuity and effectiveness of care delivery.
Progress Monitoring: Regularly assess our elderly couple’s cognitive function, behavioral symptoms, and response to interventions to evaluate the effectiveness of care strategies and identify areas for adjustment or modification. Use standardized assessment tools, behavioral observations, and family feedback to monitor changes in their condition, optimize their care plans, and enhance their overall well-being and quality of life.
Requirements:

Certification as a registered nurse with experience in geriatric nursing, dementia care, or memory support.
Proficiency in providing specialized memory care interventions and behavioral symptom management techniques for individuals with dementia, including familiarity with non-pharmacological approaches and therapeutic activities.
Strong communication and interpersonal skills, with the ability to engage and collaborate effectively with elderly individuals with dementia, their families, and interdisciplinary healthcare teams.
Compassion, empathy, and a genuine commitment to promoting the dignity, autonomy, and well-being of individuals living with dementia, supporting them and their families with sensitivity, respect, and compassion.
