Project Overview:
We are seeking skilled nurses to join our caregiving team on a contractual basis, with a focus on providing specialized care for a 55-year-old man with a terminal brain tumor. This role involves delivering supportive care, engaging the patient in cognitive stimulation activities, and offering emotional support to both the patient and his family caregivers as they navigate the challenges of terminal illness.

Responsibilities:

Supportive Care: Provide compassionate and holistic care to the patient, focusing on pain management, symptom relief, and promoting comfort and dignity throughout the end-of-life journey. Collaborate with the healthcare team to develop and implement individualized care plans that address the patient’s physical, emotional, and spiritual needs, ensuring a comprehensive approach to supportive care.
Cognitive Stimulation Activities: Engage the patient in cognitive stimulation activities tailored to his preferences and abilities, aiming to maintain cognitive function, enhance mental acuity, and promote meaningful engagement and quality of life. Offer a variety of cognitive exercises, memory games, and sensory activities to stimulate the mind and provide opportunities for social interaction and enjoyment.
Emotional Support: Provide emotional support and counseling to the patient and his family caregivers, offering empathy, validation, and a listening ear during times of distress, anxiety, and uncertainty. Offer coping strategies, relaxation techniques, and mindfulness exercises to help manage emotional distress and promote psychological well-being for both the patient and his loved ones.
End-of-Life Planning: Facilitate discussions about end-of-life preferences, advance care planning, and goals of care with the patient and his family caregivers, ensuring their wishes and values are respected and honored. Assist with coordinating palliative care services, hospice care, and other supportive resources to optimize comfort and quality of life during the terminal phase of illness.
Family Education: Provide education and guidance to family caregivers on how to best support the patient’s physical and emotional needs, including techniques for providing comfort, managing symptoms, and communicating effectively with healthcare providers. Offer resources, referrals, and ongoing support to help family caregivers navigate the caregiving journey with confidence and resilience.
Collaboration: Work collaboratively with the interdisciplinary healthcare team, including physicians, social workers, spiritual care providers, and hospice professionals, to ensure coordinated and seamless care for the patient and his family caregivers. Communicate effectively, share pertinent information, and participate in care conferences to promote continuity of care and holistic support for all involved.
Progress Monitoring: Regularly assess the patient’s symptoms, emotional well-being, and response to supportive interventions, adjusting care plans as needed to address changing needs and optimize comfort and quality of life. Use standardized assessment tools, patient-reported outcomes, and caregiver feedback to evaluate the effectiveness of interventions and tailor care to the patient’s evolving needs and preferences.
Requirements:

Certification as a registered nurse with experience in palliative care, hospice care, or oncology nursing.
Proficiency in providing supportive care, cognitive stimulation activities, and emotional support for patients with terminal illnesses, including knowledge of symptom management, end-of-life care principles, and family-centered approaches to care.
Experience in facilitating difficult conversations, supporting patients and families through grief and loss, and promoting open communication and shared decision-making in end-of-life care planning.
