Project Overview:
We are seeking an experienced nursing team specializing in palliative care and caregiver support to provide comprehensive assistance for our loved one and his family caregiver. This project involves delivering palliative care services to the patient at home, while also offering respite services, emotional support, and education on self-care to alleviate the burden on the family caregiver.
Responsibilities:
– **Palliative Care for the Patient:**
– Deliver compassionate and holistic palliative care services to the patient at home, focusing on pain and symptom management, emotional support, and maintaining dignity and quality of life.
– Collaborate with the patient, family members, and other healthcare providers to develop and implement a personalized care plan that aligns with the patient’s preferences, goals, and values.
– Monitor the patient’s condition closely, assess his symptoms regularly, and adjust interventions as needed to ensure optimal comfort and well-being.
– **Respite Services for the Caregiver:**
