About the Client:
We are a dedicated family committed to providing compassionate care for our 65-year-old family member who is in the terminal stage of liver cirrhosis. He is experiencing severe pruritus and abdominal ascites, which significantly impact his comfort and quality of life. Our primary focus is to ensure his symptoms are managed effectively and to support our family caregivers in maintaining his dignity and comfort during this difficult time.
Project Overview:
We are seeking an experienced nursing team specializing in palliative care to provide comprehensive support for our family member. This project involves managing symptoms associated with terminal liver cirrhosis, providing palliative care to enhance comfort, and offering support and education to family caregivers.
Responsibilities:
Symptom Management:
Develop and implement a symptom management plan to address severe pruritus and abdominal ascites.
Administer medications to relieve pruritus, such as antihistamines, bile acid sequestrants, or topical treatments.
Manage abdominal ascites through diuretic therapy, dietary adjustments (e.g., low-sodium diet), and potential paracentesis to relieve discomfort.
Palliative Care:
Provide holistic palliative care focused on improving quality of life and alleviating distressing symptoms.
Offer pain management strategies, including pharmacologic and non-pharmacologic interventions, to ensure the patient remains as comfortable as possible.
Address any additional symptoms that arise, such as nausea, fatigue, or dyspnea, with appropriate interventions.
Emotional and Psychological Support:
Provide emotional and psychological support to the patient to help him cope with the challenges of terminal illness.
Facilitate conversations about end-of-life wishes and advance care planning, ensuring the patient’s preferences and values are respected.
Support for Family Caregivers:
Educate family caregivers on symptom management techniques and provide guidance on how to care for their loved one effectively.
Offer respite care to give family caregivers a break and reduce burnout, ensuring they can continue to provide quality care.
Provide emotional support and counseling for family caregivers to help them navigate the emotional challenges of caring for a terminally ill loved one.
Coordination of Care:
Collaborate with other healthcare providers, including the patient’s primary care physician, hepatologist, and palliative care specialists, to ensure coordinated and comprehensive care.
Monitor the patient’s condition regularly and adjust the care plan as needed based on symptom progression and the patient’s comfort.
Requirements:
Qualifications and Experience:
Certification or extensive experience in palliative care nursing, with a focus on managing symptoms in terminal liver cirrhosis.
Proficiency in pain management, pruritus relief, and abdominal ascites management.
Skills and Competencies:
Strong communication and interpersonal skills to provide compassionate care and support to both the patient and family caregivers.
Knowledge of evidence-based palliative care practices and symptom management strategies for terminal liver disease.
