About the Client:
We are dedicated to providing compassionate care for a 65-year-old man diagnosed with terminal liver cirrhosis. His condition is complicated by symptoms of ascites and hepatic encephalopathy, significantly impacting his quality of life and daily functioning.
Project Overview:
We are seeking a skilled nursing team to provide comprehensive support for our client with terminal liver cirrhosis. The project involves implementing diuretic therapy, dietary restrictions, and supportive care interventions to manage symptoms effectively and improve his overall well-being.
Responsibilities:
– **Diuretic Therapy:**
– Administer prescribed diuretic medications as directed by the healthcare provider to reduce fluid accumulation and alleviate symptoms of ascites.
– Monitor the client’s fluid balance, electrolyte levels, and response to diuretic therapy, adjusting dosages as necessary to optimize therapeutic outcomes.
– **Dietary Restrictions:**
– Collaborate with a registered dietitian to develop and implement dietary restrictions tailored to the client’s condition, including restrictions on sodium and fluid intake to manage ascites and prevent fluid retention.
– Educate the client and his family members on adhering to dietary recommendations, emphasizing the importance of compliance for symptom management and overall health.
– **Supportive Care:**
– Provide supportive care interventions to address symptoms of hepatic encephalopathy, such as confusion, cognitive impairment, and altered consciousness.
– Implement strategies to promote safety and prevent falls or injuries due to impaired cognitive function, including environmental modifications and close supervision.
– **Collaboration and Monitoring:**
– Collaborate closely with the interdisciplinary healthcare team, including physicians, nurse practitioners, and allied health professionals, to coordinate care and optimize treatment outcomes.
– Monitor the client’s condition closely, assessing for changes in symptoms, disease progression, and response to therapeutic interventions, and communicate findings to the healthcare team promptly.
Requirements:
– **Qualifications and Experience:**
– Registered nurses (RNs) with experience in caring for patients with liver cirrhosis and related complications, preferably in a palliative care or hospice setting.
– Familiarity with the management of ascites, hepatic encephalopathy, and diuretic therapy in patients with advanced liver disease.
– **Skills and Competencies:**
– Proficiency in administering diuretic medications and monitoring for potential adverse effects, such as electrolyte imbalances and renal dysfunction.
– Knowledge of dietary management principles for patients with liver cirrhosis, including the ability to provide education and support to promote dietary adherence.
