About the Client:
We are a supportive family seeking specialized care and support for our 70-year-old father who is living with Parkinson’s disease and experiencing freezing of gait. Our primary goal is to enhance his mobility, reduce the risk of falls, and improve his overall quality of life, while providing support and resources for our family caregivers.
Project Overview:
We are seeking an experienced healthcare professional with expertise in Parkinson’s disease management to provide comprehensive care for our father. This project entails medication adjustments, physical therapy, and assistive devices to address freezing of gait and promote mobility and independence.
Responsibilities:
Conduct medication assessments and adjustments under the guidance of our father’s neurologist, optimizing his medication regimen to manage Parkinson’s symptoms, including freezing of gait, and minimize motor fluctuations and dyskinesias.
Implement physical therapy interventions tailored to our father’s needs and abilities, focusing on gait training, balance exercises, and mobility aids to improve his walking ability, reduce freezing episodes, and enhance his overall functional mobility and independence.
Recommend and facilitate the use of assistive devices and mobility aids, such as canes, walkers, or specialized footwear, to provide stability, support, and safety during ambulation and activities of daily living, while minimizing the risk of falls and injuries.
Educate our family caregivers on strategies for fall prevention, safe mobility techniques, and environmental modifications to create a home environment that supports our father’s mobility, independence, and safety.
Collaborate with our father’s multidisciplinary healthcare team, including neurologists, physical therapists, occupational therapists, and speech-language pathologists, to ensure coordinated and holistic care that addresses his physical, cognitive, and psychosocial needs.
Requirements:
Experience in Parkinson’s disease management, with a comprehensive understanding of motor and non-motor symptoms, medication therapies, and rehabilitation interventions to optimize functional mobility and quality of life for individuals living with Parkinson’s disease.
Knowledge of freezing of gait pathophysiology and evidence-based strategies for its management, including medication adjustments, physical therapy techniques, and assistive devices to enhance gait performance, reduce fall risk, and improve overall mobility and independence.
Excellent communication and interpersonal skills, with the ability to collaborate effectively with healthcare providers, patients, and family caregivers to develop personalized care plans, address concerns, and facilitate informed decision-making and goal setting.
Empathy, patience, and a person-centered approach to care that recognizes our father’s unique experiences, preferences, and goals, while fostering a supportive and empowering caregiving relationship.
