Project Overview:

Responsibilities:

Coordinate care plans that incorporate medication management, psychotherapy, and complementary therapies to optimize our family member’s mental and physical health outcomes.
Medication Management: Provide comprehensive medication management to ensure our family member receives the appropriate medications for managing her chronic insomnia, anxiety disorder, and chronic pain. Monitor medication effectiveness, side effects, and interactions, adjusting dosages and treatment plans as needed under the guidance of healthcare professionals to optimize symptom control and minimize adverse effects.
Cognitive-Behavioral Therapy (CBT): Offer cognitive-behavioral therapy interventions to address our family member’s insomnia, anxiety, and pain. Implement evidence-based CBT techniques to help her identify and challenge negative thought patterns, develop coping skills for managing stress and anxiety, and improve sleep hygiene and pain management strategies, empowering her to regain control over her symptoms and improve her overall quality of life.
Collaboration: Collaborate closely with healthcare providers, including psychiatrists, psychologists, pain specialists, and physical therapists, to ensure coordinated and holistic care for our family member. Communicate regularly with the interdisciplinary team to provide updates on her progress, address concerns, and adjust care plans as needed, ensuring she receives the best possible care and support tailored to her individual needs and preferences.

Requirements:

Certification as a registered nurse with experience in mental health nursing, pain management, or integrative healthcare.
