Project Overview:
We are seeking skilled nurses to join our caregiving team on a contractual basis, with a focus on providing specialized care for a 60-year-old woman with chronic kidney disease (CKD) undergoing peritoneal dialysis at home. This role involves delivering medication management, cognitive-behavioral therapy (CBT), and support for managing anxiety and sleep disturbances to promote our patient’s emotional well-being and improve her quality of life.

Responsibilities:

Medication Management: Provide comprehensive medication management to our patient, ensuring adherence to prescribed medications and monitoring for effectiveness and potential side effects. Collaborate with our patient’s healthcare providers to adjust medication regimens as needed to address anxiety and sleep disturbances, optimizing her treatment plan for improved symptom management and overall well-being.
Cognitive-Behavioral Therapy (CBT): Implement evidence-based cognitive-behavioral therapy techniques to address our patient’s anxiety and sleep disturbances. Conduct structured CBT sessions to help our patient identify and challenge negative thought patterns, develop coping strategies for managing anxiety, and improve sleep hygiene practices to promote restful sleep and enhance her emotional well-being.
Emotional Support: Offer empathetic support and guidance to our patient as she navigates the challenges of living with chronic kidney disease and undergoing peritoneal dialysis. Provide a compassionate presence, active listening, and encouragement to help our patient cope with anxiety and sleep disturbances, fostering a sense of reassurance and emotional resilience.
Collaboration: Collaborate closely with our patient’s healthcare team, including nephrologists, dialysis nurses, psychologists, and other specialists, to ensure coordinated and holistic care. Communicate regularly with the interdisciplinary team to share information, discuss treatment goals, and address any emerging needs or concerns, promoting continuity and effectiveness of care delivery.
Progress Monitoring: Regularly assess our patient’s anxiety levels, sleep patterns, and response to interventions to evaluate the effectiveness of medication management and cognitive-behavioral therapy. Use standardized assessment tools, sleep diaries, and patient feedback to monitor changes in our patient’s symptoms, adjust intervention strategies as needed, and optimize her emotional well-being and quality of life.
Requirements:

Certification as a registered nurse with experience in nephrology nursing, home healthcare, or mental health nursing.
Proficiency in medication management for individuals with chronic kidney disease and peritoneal dialysis, including knowledge of pharmacological interventions for managing anxiety and sleep disturbances.
Experience in delivering cognitive-behavioral therapy (CBT) techniques to address anxiety and sleep disturbances, with a strong understanding of CBT principles and therapeutic interventions.
Strong communication and interpersonal skills, with the ability to provide compassionate care and support to individuals undergoing peritoneal dialysis, fostering trust and rapport, and collaborating effectively with interdisciplinary healthcare teams.
