About the Client:
We are a caring and devoted family dedicated to the well-being and safety of our beloved grandmother, who is 85 years old and living with Alzheimer’s disease. We are deeply concerned about her wandering tendencies, and our primary focus is to ensure her safety and well-being while also providing support for our family caregivers.

Project Overview:
We are seeking experienced healthcare professionals who specialize in Alzheimer’s care and dementia management to provide comprehensive support for our grandmother and our family caregivers. This project entails implementing tailored interventions, including safety measures, cognitive stimulation activities, and caregiver support, to address her wandering tendencies and ensure her well-being.

Responsibilities:

Conduct a thorough assessment of our grandmother’s wandering tendencies and cognitive abilities to develop a personalized care plan that addresses her unique needs and minimizes the risk of elopement.
Implement safety measures within her living environment to prevent wandering and ensure her safety, including installing door alarms, locks, and other security devices, as well as removing potential hazards.
Provide cognitive stimulation activities and structured routines to engage our grandmother and promote mental stimulation, including reminiscence therapy, sensory stimulation, and simple tasks that align with her interests and abilities.
Offer support and education to our family caregivers on how to effectively manage wandering behaviors, including techniques for redirection, supervision strategies, and communication strategies to help prevent elopement and ensure our grandmother’s safety.
Collaborate closely with our grandmother’s physician, neurologist, and other healthcare providers to ensure coordinated care and optimize treatment outcomes.
Educate and support our family in understanding Alzheimer’s disease, its progression, and how to best support our grandmother’s well-being and safety at home.
Requirements:

Certification or extensive experience in Alzheimer’s care, dementia management, or related fields, with expertise in addressing wandering tendencies and safety concerns in individuals with Alzheimer’s disease.
Proficiency in implementing safety measures and environmental modifications to prevent wandering and ensure the safety of individuals with Alzheimer’s disease, including familiarity with door alarms, locks, and other security devices.
Experience in providing cognitive stimulation activities and structured routines to engage individuals with Alzheimer’s disease and promote mental stimulation, with a focus on personalized interventions tailored to their interests and abilities.
Knowledge of caregiver support techniques and resources for families caring for individuals with Alzheimer’s disease, including education, respite care, and support groups.
Excellent communication skills and a compassionate demeanor, fostering trust and collaboration with our grandmother and her family caregivers, as well as interdisciplinary healthcare teams involved in her care.
