Project Overview:
We are seeking compassionate and experienced nurses to provide comprehensive care for our family member who requires dialysis due to ESRD. This project entails a 6-month contract with the possibility of extension based on the evolving needs of his health and treatment.
Responsibilities:
Provide attentive and specialized care tailored to the needs of a patient with ESRD.
Coordinate and manage dialysis treatments, ensuring adherence to the prescribed schedule.
Administer medications and follow prescribed treatments as directed by healthcare professionals.
Implement dietary management plans to support kidney health and overall well-being.
Monitor the patient’s overall health conditions and vital signs to detect and address complications.
Collaborate closely with family members and other caregivers to maintain a cohesive and supportive care plan.
Requirements:
Certification as a registered nurse with a minimum of 3 years of relevant experience in nephrology or dialysis care.
Proficiency in coordinating dialysis treatments and managing related healthcare needs.
Strong understanding of dietary requirements and restrictions for patients with ESRD.
Ability to work independently and as part of a team in a home environment, demonstrating flexibility and adaptability to the unique needs of the patient.
Excellent communication skills and a compassionate demeanor, fostering a supportive and collaborative relationship with the family and other caregivers.

Condition: The patient is living with end-stage renal disease and requires regular dialysis treatments to manage his condition. He needs comprehensive and ongoing care to support his health and well-being.
Care Plan:
Dialysis Coordination: Schedule and manage dialysis treatments, ensuring the patient attends all sessions and follows pre- and post-dialysis care protocols.
Medication Administration: Ensure timely administration of prescribed medications, monitoring for side effects and effectiveness.
Dietary Management: Implement and monitor dietary plans tailored to the patient’s needs, focusing on kidney-friendly foods and fluid restrictions.
