Client Overview:
We are a caring family dedicated to supporting our 70-year-old loved one who is recovering from a stroke. Our family member has experienced hemiplegia and aphasia as a result of the stroke, which have significantly impacted their mobility and communication abilities. As their family, our primary goal is to facilitate their rehabilitation and help them regain function and independence to the fullest extent possible. We are seeking skilled nursing assistance to provide comprehensive care and support tailored to their specific needs, including physical therapy, speech therapy, and assistive technology to promote their recovery and improve their quality of life.

Project Overview:
We are seeking an experienced nurse to join our caregiving team on a contractual basis, with a focus on providing specialized care for our 70-year-old family member recovering from a stroke with hemiplegia and aphasia. This role involves providing physical therapy, speech therapy, and assistive technology to help them regain function and improve communication skills, facilitating their rehabilitation and enhancing their overall well-being.

Responsibilities:

Physical Therapy: Implement individualized physical therapy interventions to improve our family member’s mobility, strength, and coordination following the stroke. Utilize evidence-based techniques, therapeutic exercises, and functional training activities to address hemiplegia and promote motor recovery, facilitating their ability to perform daily tasks and activities independently.
Speech Therapy: Provide targeted speech therapy interventions to address our family member’s aphasia and communication difficulties post-stroke. Conduct comprehensive assessments to identify specific speech and language impairments, and develop personalized therapy plans focusing on improving verbal expression, comprehension, and cognitive-linguistic skills, enhancing their ability to communicate effectively with others.
Assistive Technology: Recommend and facilitate the use of assistive technology devices and communication aids to support our family member’s communication needs. Explore options such as speech-generating devices, augmentative and alternative communication (AAC) systems, and mobile applications designed to enhance their ability to express themselves and participate in social interactions, promoting their independence and engagement in meaningful activities.
Progress Monitoring: Regularly assess our family member’s progress in physical and speech therapy interventions, tracking improvements in mobility, communication skills, and overall functional abilities. Collaborate with the interdisciplinary rehabilitation team to adjust treatment plans as needed, address emerging needs or challenges, and optimize the effectiveness of therapy interventions, ensuring our loved one receives the best possible care and support.
Education and Training: Provide education and training to our family member and their caregivers on techniques for maximizing recovery, promoting independence, and using assistive technology effectively. Empower them with knowledge and skills to continue therapy exercises and communication strategies at home, fostering ongoing progress and participation in rehabilitation activities outside of formal therapy sessions.
Collaboration: Collaborate closely with other healthcare professionals, including physical therapists, speech-language pathologists, occupational therapists, and rehabilitation specialists, to ensure coordinated and integrated care for our family member. Communicate regularly with the interdisciplinary team to share information, discuss treatment goals, and address any concerns or barriers to recovery, promoting continuity and effectiveness of care.
Family Support: Offer emotional support and guidance to our family as they navigate the challenges of supporting a loved one recovering from a stroke. Provide encouragement, resources, and referrals to support groups or community organizations that offer assistance and camaraderie to individuals and families affected by similar conditions, fostering resilience and mutual support within the family unit.
Requirements:

Certification as a registered nurse with experience in stroke rehabilitation, physical therapy, or speech therapy.
Proficiency in providing physical therapy and speech therapy interventions for individuals recovering from strokes, including those with hemiplegia and aphasia.
Knowledge of assistive technology devices and communication aids for individuals with communication impairments, with the ability to recommend and facilitate their use effectively.
Strong communication and interpersonal skills, with the ability to provide compassionate care and support to individuals and families facing the challenges of stroke recovery.
