Our primary goal is to manage his symptoms effectively and enhance his comfort and well-being during this challenging time.

Project Overview:
We are seeking a nursing team with expertise in palliative care to support our client in managing symptoms associated with terminal liver cirrhosis. This project involves implementing diuretic therapy, dietary restrictions, and supportive care measures to alleviate symptoms, improve quality of life, and provide compassionate end-of-life care.

Responsibilities:

– **Symptom Management:**
– Administer diuretic medications as prescribed to reduce fluid retention and alleviate symptoms of ascites.
– Monitor the client’s fluid balance, renal function, and electrolyte levels closely to ensure the safe and effective use of diuretics.

– **Dietary Restrictions:**
– Collaborate with a registered dietitian to develop a nutrition plan tailored to the client’s condition, focusing on sodium restriction and protein moderation to manage ascites and hepatic encephalopathy.
– Educate the client and his family about dietary modifications and fluid restrictions to support symptom management and optimize nutritional status.

– **Supportive Care:**
– Provide holistic care and emotional support to the client and his family, addressing their physical, psychological, social, and spiritual needs.
– Offer counseling and guidance to the client and his family regarding end-of-life care preferences, advance care planning, and goals of care discussions.

– **Symptom Assessment and Monitoring:**
– Conduct regular assessments of the client’s symptoms, including ascites, hepatic encephalopathy, pain, and overall comfort level.
– Monitor for signs of disease progression or complications, such as hepatic coma or gastrointestinal bleeding, and intervene promptly to manage symptoms and maintain stability.

– **Communication and Coordination:**
– Facilitate open and honest communication between the client, his family, and the healthcare team, ensuring that everyone is informed and involved in decision-making.
– Coordinate care transitions and collaborate with other healthcare providers, including physicians, hospice teams, and social workers, to ensure continuity of care and comprehensive support for the client and his family.

Requirements:
