Project Overview:
We are seeking experienced healthcare professionals who specialize in palliative care to provide comprehensive support for our father and our family caregivers. This project entails a commitment to delivering compassionate care and symptom management for our father, while also offering emotional support and guidance to our family members.
Responsibilities:
Provide palliative care to alleviate pain and discomfort associated with liver cirrhosis, focusing on effective symptom management for ascites, hepatic encephalopathy, and pruritus.
Offer emotional support and counseling to our father and our family caregivers, helping them navigate the challenges and complexities of terminal illness and end-of-life care.
Collaborate closely with our father’s medical team, including primary care physicians, specialists, and hospice providers, to ensure coordinated and holistic care that addresses his physical, emotional, and spiritual needs.
Educate and empower our family caregivers by providing guidance on how to best care for our father, manage symptoms, and maintain their own well-being during this challenging time.
Facilitate discussions about goals of care, advance care planning, and end-of-life preferences, ensuring that our father’s wishes are respected and honored.
Requirements:
Certification or experience in palliative care or hospice care, with a demonstrated understanding of symptom management and end-of-life care for patients with terminal liver cirrhosis.
Strong communication and counseling skills, with the ability to provide compassionate support and guidance to patients and their families during difficult times.
Experience in collaborating with interdisciplinary healthcare teams to deliver holistic and patient-centered care for individuals with terminal illnesses.
Knowledge of community resources and support services available for patients and families facing end-of-life care, including bereavement support and counseling.
A compassionate demeanor and a commitment to providing dignified and compassionate care to patients and their families during the end-of-life journey.
