Offer compassionate support and comfort measures to enhance the patient’s quality of life and ensure dignity and respect in end-of-life care.

Provide education on end-of-life care, facilitate open communication, and assist with advance care planning to ensure alignment with the patient’s wishes and preferences.
Collaboration: Work closely with the patient’s multidisciplinary care team, including physicians, nurses, social workers, and spiritual counselors, to ensure coordinated and holistic care. Communicate effectively with team members, share relevant information, and participate in interdisciplinary care conferences to develop and implement individualized care plans that address the patient’s unique needs and goals.
Progress Monitoring: Regularly assess the patient’s symptoms, treatment response, and overall well-being to evaluate the effectiveness of palliative care interventions.
